The Department of Human Services announced a data breach which
included personal information in Dallas County.
On Friday, DHS said 4,784 people's information was compromised due to
a breach on Nov. 25, 2019. The custodial staff "mistakenly emptied
boxes containing documents to be shredded into the office's garbage
dumpster," DHS said. Due to this, the department is uncertain which
documents were included in the disposal. DHS has notified all parties
who may have been affected since their documents were not shredded
before being disposed of.
The breach includes "name, mailing address, date of birth, social
security number, driver's license number, banking or wage information,
disability information, medical information, receipt of Medicaid,
mental health information, provider information, substance abuse
information, illegal drug use information or prescription drug
information," according to DHS.
An internal investigation was launched on Dec. 2, 2019 and found the
custodial employee was unaware of the confidential aspect of the
papers disposed of.
